Responsibilities: Works with Crisis Program Manager to ensure QA, other designated duties, are conducted on consumer files and related services. Meets with supervisor weekly for supervision and/or consultation. Services will be provided in the community, emergency rooms, or in a community behavioral agency Services will be provided in the community, in a consumers home or in one of the program locations they may include crisis intervention or services provided as follow up to crisis intervention, assisting with applying for benefits, activities of daily living, psychosocial support, service coordination, linkage, transportation, accessing community resources and/or housing/housing retention and connecting to any needed, available and eligible services up to including medical, mental health, vocational, education, financial, etc. Assists in referral and provision of: Intake Family treatment Medication management Peer support, psychological assessment Therapeutic psychoeducation including Illness Management and Recovery, and referrals to ITA or LRA services. Provides continuity of care in support of established client/therapist relationships and best practices, coordinates care with providers internally and outside of the agency with a Release of Information. Is responsible for maintaining safety precautions and constructive, professional relationships with Supportive Housing, Crisis, and Outpatient Team members. Completes appropriate documentation for services provided within required timeframes. Establishes and maintains working relationships with community resources, coordinates consumer treatment with other providers, and initiates referrals to medical providers and other community resources when appropriate. Reports and documents all services in the Electronic Medical record as well as other contractually required systems. Maintains contacts on google spreadsheet and enrolls eligible individuals in Clarity HMIS system. Documentation will be completed no later than 5 pm the following day of the service. Coordinate important services and observation with other assigned providers within and outside of the agency. Attend all required individual supervision, group supervision, all staff meetings, clinical training, and other trainings asked to attend by management. Attend any community meetings asked to attend by supervisor. Promote Catholic Charities SUD program while meeting with other resources while in the community. Search for, create report with, and secure landlords within the community willing to participate in housing Supportive Housing clients.
