POSITION SUMMARY:

The case manager coordinator will assist in program development, marketing, staff training and service monitoring for the clinic’s case management program. This position will take the lead role in coordinating the activities of the case manager team as well as collecting and reporting all necessary data to the supervisor. This position is responsible for initial assessment, referral for evidence-based practices, and re-assignment of cases to the ongoing case manager, as well as assisting in the facilitation of case management team meetings.

 

EDUCATION AND EXPERIENCE:

  • A minimum of a Bachelor’s degree in a mental health, or a related field from an accredited institution required.

  • A Master’s degree in social work, counseling or psychology, with appropriate state licensure is preferred.

  • A minimum of 3 years’ supervised experience in examination, evaluation & treatment of minors and families required.

  • A minimum of 2 years’ case management experience preferred.

     

    COMPETENCIES AND ABILITIES:

  • Knowledge of various systems operating in Wayne County (i.e. Community Mental Health, Department of Social Services, Juvenile Court, Probate Court, schools, etc).

  • Ability to skillfully engage with children, adolescents, and their families.

  • General knowledge of children’s mental health and treatment practices.

  • Ability to interpret case management services to various bureaucracies.

  • Ability to provide assistance to families in collecting information to complete a full financial for billing purposes.

  • Ability to negotiate alliances among various agencies to establish trust and to articulate mutual interests to further client interests.

  • Ability to always represent the agency in a professional manner.

  • Ability to effectively communicate verbally and in writing.

  • Must respect the confidentiality of all program participants.

  • Ability to make critical decisions independent of immediate supervision.

  • Ability to plan ahead and organize time effectively.

  • Ability to manage multiple priorities, function independently and demonstrate good follow through.

  • Must have a working knowledge of all SFS programs.

  • Must be able to relate to all members of the community regardless of race, age, gender, ethnic origin, etc.

  • Ability to participate in a multi-disciplinary approach to treatment and case management.

  • Ability to maintain professional and ethical standards.

  • Ability to use standard office equipment including a computer.

     

    DUTIES AND RESPONSIBILITIES:

  • Provide anticipatory guidance to other professionals within the realm of case management. This includes training, technical assistance, and monitoring and coordination of assessments for children and families.

  • Assist in identification and reduction of cross-system barriers.

  • Monitor program referrals and assignment of caseloads to the case management program.

  • Identify and work closely with supervisor and staff on program development and client needs.

  • Provide direct services to clients and families.

  • Meet productivity expectation as assigned by program supervisor

  • Develop/ maintain liaison relationships with various community facilities.

  • Develop, in a timely manner, a written client comprehensive assessment and service plan, which identifies the needs of the client/family.

  • Assist the family in advocacy for the development and provision of appropriate community services for the client.

  • Facilitate the out-of-home placement of a client when necessary.

  • Work flexible hours to accommodate the needs of the program and clients.

  • Responsible for keeping client files in order and up to date.

  • Participate in and contribute to staff case reviews and staff training in collaboration with mental health therapist.

  • Cooperate in any investigation related to personnel, licensing, accreditation, or other circumstances.

  • Responsible for working with team members, supervisor, child, families, and community contacts in a manner that is conducive to the philosophy and mission of the program and agency.

  • Attend mandatory agency meetings and training.

  • Assist in finding and directing clients in case of an emergency such as a tornado or fire.

  • Other duties to be performed as assigned by supervisor.

     

    LICENSING AND OTHER REQUIREMENTS:

  • Must have reliable mode of transportation Must possess a current valid State of Michigan chauffeur’s driver’s license and automobile insurance that is insurable. Must meet SFS and our insurance carrier guidelines for the transportation of children/ clients.
